Using Data to Drive Outcomes: The Key to Personalized Vascular Access Care

A clinician’s quest to advance practice for the betterment of patients can be a challenge when they are not armed with the tools, resources, or funding to fuel change. Data is often seen as an IT project or for publication and not necessarily a means for patient improvement. This presentation looks at how available data within the healthcare setting can become a valuable asset in the pursuit to drive change and deliver personalized care. From identifying an issue and establishing a planned method of collection, this presentation will discuss the factors that drive decision making, and how data can help strengthen a position for effecting change, ultimately resulting in a compassionate approach specific to each patient.

  • Objectives for this presentation are:

Identify at least three historical studies which had a major impact on vascular access.
Name at least three significant vascular access practice changes.
Compare standardized care versus personalized care.
Consider disease and co-morbidity impacts on vascular access.
Identify mechanisms to drive change through data.
Discuss ways to collect data and consider tools available.
Consider necessary components of data tool development.
